What's Confirmed and What Isn't
What we know: Onyxia's Lair returns at 40 players, level 60, in its original Dustwallow Marsh location. What we don't know yet: whether the encounter's mechanics change, whether the original loot table is retained as-is or reworked, whether the attunement chain is identical to Classic's, and whether the fight receives additional itemization from Forever's broader loot overhaul. Guides built entirely on the Vanilla Onyxia fight shouldn't be treated as a permanent Forever guide until these are confirmed.
The Onyxia Attunement, Then and Now
In original Classic, entering Onyxia's Lair required a long quest chain ending in the Drakefire Amulet, sending players through Burning Steppes, Winterspring, and a fight inside Upper Blackrock Spire before the amulet was earned. That requirement was later removed in other eras of WoW, but Classic's 2019 relaunch restored the full chain.
